NHS 111 Wales - Check Your Symptoms

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Care Navigation

Our reception team are trained in Care Navigation. This means they are able to direct patients to the most appropriate healthcare service for their needs.

Depending on your symptoms or query, this may include services such as opticians, minor injuries units, pharmacies, dentists, or A&E.

To help ensure you receive the right care as quickly as possible, our receptionists will ask for a brief description of your issue. This allows patients to be appropriately triaged and, where suitable, signposted to another service. By doing this, we can help keep more GP, Nurse, and Healthcare Assistant appointments available for patients who need them most.

We understand that it can be frustrating when you are feeling unwell. However, our reception staff are following practice procedures and always aim to help every patient to the best of their ability.

Please remember that we operate a zero-tolerance policy towards abusive or aggressive behaviour directed at our staff.

Practice Nurse/ HCA appointments

We currently have 2 practice nurses Debbie and Tina, who are highly trained in monitoring long-term chronic conditions such as asthma, COPD, Diabetes, heart and vascular disease, blood pressure monitoring and warfarin. All patients with these conditions are advised to respond to recall letters and attend reviews with our practice nurses.

Our nurses are also fully trained in -

  • HRT and Contraception monitoring
  • Smears
  • Travel advice and immunisations
  • Child health immunisations
  • Anticoagulant reviews

We also have a Health care assistant Lynn who can help with -

  • Take urgent blood tests
  • Routine ECG's
  • routine injections

Routine appointments to see the nurse/HCA are available to be booked via telephone

Cancelling or changing an appointment

We still get a large ammount of appointments not attended but not cancelled. If you are unable to attend your appointment please contact the surgery to cancel or rearrange.

To cancel your appointment:

  • Phone us on 01492 550430 (Penrhyn), Monday to Friday from 8am to 6:30pm
  • reply CANCEL to your appointment reminder text message

Home visits

If you are housebound and need an appointment, we are able to do a home visit. A clinician will contact you first, triage the issue and either plan to come out or request help from other services such as the community advanced nurse practitioners or the community district nurses.

Care homes are encouraged to request a visit from the community advanced nurse practitioners initially who will triage and see if a visit from them is suitable.

To request a home visit, it’s helpful if you phone the practice before 11am.
