If you would like to register as a patient of Penrhyn Bay or Deganwy Medical Centre, firstly you will need to check you are in our catchment area.

To register with the surgery:
- fill out an online registration form
- visit reception from 8am to 6pm and collect a paper registration form
The form requires your NHS number, you can get this from a repeat slip, hospital letter or by contacting your previous surgery.
Once the forms are complete, as part of the registration process, you will be asked to attend a "new patient health check" with our Health Care Assistant. Once this is complete you will be fully registered and able to book to see a GP.
Please make sure you request a month supply of medication from your previous surgery before registering, your medical records can take a while to come over and it takes time to process and get medication on the system safely.
Temporary patients
If your are in the area for less that 3 months you can register as a temporary patient for the duration of your stay, please visit the reception and request a "temporary resident form".
